Join us in North Minneapolis Tuesday, November 11th for the next CHAT (Community Health and Advocacy Talks): “From Population Health to Patient Care” presented by Veronica Svetaz MD, MPH, the medical director of Aqui Para Ti at the Hennepin County Medical Center.
Dr. Svetaz will reflect on the creation of a “health inequity-proof” model of care. She will pay particular attention to the role of a healer when caring for populations made vulnerable by avoidable social determinants of health.
CHAT presentations are open to the public and held in the evenings on the second Tuesday of every month at the UROC (Urban Research and Outreach Center) – 2001 North Plymouth Avenue Minneapolis, MN 55411.
Each CHAT session consists of:
- 5.30- 6 pm – light dinner/snacks, meet & greet
- 6 – 7 pm – presentation
- 7-7.30 pm – networking
